SOMA Physical and Occupational Therapy commits to protecting your PHI to our best possible ability. We commit to contacting you if there is a breach in our electronic medical record system. Please note that your PHI may be incidentally disclosed when using SOMA Physical and Occupational Therapy services. For instance, given the open treatment area, other patients may observe/hear discussions regarding your PHI. The HIPPA Privacy Rule allows for such incidental disclosures. We will contact you if any terms of this notice change.
